Director Exit Questionnaire

This form is designed for use by a homeowners’ association to obtain constructive information from an outgoing member of the association’s board of directors. The purpose is to enable the association to get feedback from a former director to evaluate and utilize for consideration of ways to improve the operations of the association’s board of directors.
